Dear Ms.Tavares:
REFERENCE: Dr. Sarah Feaver MD,
Breach of Operator Licensing and Vehicle Control Regulation, Alberta Traffic Safety Act, Alberta regulation 320/ 2002, Section 16 [2].
Further to our letter of April 6, 2018 which informed you and the Minister of Transportation, that Dr. Sarah Feaver had refused to provide Mr. XXXX XXXXX

1) a medical examination
2) a completed Medical Examination for Motor Vehicle Operators form.

It has been alleged to us by Mr. XXXX XXXX that she would not provide this to him for the reason that he scored low on a DriveABLE test.
Now your office, Linda Gordulic, Reviewing Officer, Driver Fitness and Monitoring, Alberta Transportation, in letter dated May 29, 2018, is requiring a completed Medical Examination for Motor Vehicle Operators Form from Mr. XXXXXXX.
Please advise.
Thank you.
We would be grateful to be in receipt of a response concerning this matter by the 10th of June, 2018
Yours truly,
