Fatality Inquiry Day # 2

This morning testimony by Tim Bouwsema Registered Psychiatric
Nurse (RPN) testified to the court that he, together with
Joshua Middleton removed Lisa to the secure cell.
He said that Lisa had been agitated and acting out after she
had her IPod taken from her. He testified that he told Lisa
that she would be in seclusion room UNTIL SHE CALMED DOWN.
He was asked by Counsel for family if he had thought of returning
the IPod to her in order to ameliorate the situation, to which
he responded, NO.
He said that they lifted her up under the arms, holding back the
wrists. In an account at Villa Caritas / Alberta Hospital, an
80 year old lady’s arm was fractured while she was being “contained”
in this way.
While these two staff were carrying her, two security guards were each
holding a leg. He further DENIED that he WRAPPED her in a STRONG
BLANKET but rather placed the blanket over her to provide warmth &
comfort.
However according to a statement given to Detective Niehaus, 7 weeks
after the death, Lisa had been totally secured – as following:
WRAPPED LISA IN A STRONG BLANKET-
ARMS WRAPPED UP ON IT
FEET OPEN
FULL BODY WRAP
HEAD OPEN
Counsel told the Inquiry that a statement given by Josh Middleton
to detective Niehaus confirmed this.
WRAPPED UP IN STRONG SHEETS.

Nathan Achen testified to the Inquiry that he had been a security
guard at the time of the incident. Told the Inquiry that Lisa had
been very agitated & he had held one of hr legs when she was being
removed to the secure room.
When he left the secure room, Lisa was still STRUGGLING & MAKING NOISES.

Dallas Maybroda, Peace Officer testified that he had been summoned to
Unit 2B. His account of Lisa was in contradiction to previous testimony.
When he arrived at Unit 2B, she was JUST SITTING THERE. Said she was
very little problem, he walked along. He said she gave “passive resistance”.

Takedowns at Covenant Health Villa Caritas

Dallas Maybroda was called back to unit 2B @ 3:19. Found Lisa on her back,
testified that she looked BLUE. He immediately started concussion.
Dr.Abdullah wa present, Lillian Slashinski & other Health personnel were
present.
Maybroda called EPS @ 3:38 AM.
When police arrived, he overheard police say that Lisa had been dead for
several hours.
Mattress was not under Lisa, was in toilet room. 06/04/16
