Lisa Goltman Fatality inquiry Day # 5

Fatality Inquiry, JOLENE HASTIE, Security Officer witness

She informed the court that she had been called to Unit 8, 2B, on the
afternoon of March 25, 2013, found Lisa Goltman in the common room.
It was identified as a “duress call’, a serious call.
Said she could not remember how she was positioned but was on the
floor.
Jolene told the Inquiry that she noted a lot of blood and urine on
the floor around Lisa.
She noted Lisa’s “LIP WAS BLEEDING A LOT” & “there was a CUT AND BUMP
on her forehead”.
Jolene informed the Inquiry that she helped to place Lisa in a stretcher,
who was QUITE CO-OPERATIVE.
Staff was assisting with wounds , ice packs & face cloths.Dr. Brown did
vitals, etc.

WHY was security CALLED? She did not recall why.

Goltman family contacted police

Next witness – JOSHUA MIDDLETOWN
– psych aide – works casual on 8 2B, since August 2012.
Re: March 25, 2015 incident,
testified to the Inquiry that LISA WAS LYG ON THE COUCH, said
that there was a puddle of urine in front of her. He said he spent five
minutes engaging her to go and shower, clean up, then later said he spent
TWO MINUTES engaging her GIVING HER VERBAL DIRECTION.
Then together with another female staff proceeded to restrain Lisa.
He on Lisa’s left side, other staff on right side.
WHAT their intention to do with her was not expressed.
While staff held her by both arms, Lisa allegedly lunged forward – JOSH fell
ON HIS KNEES, FELL FORWARD ON RIGHT KNEE.
When asked if he tried to pull her off the couch, he said he did not recall.
HE COULD NOT RECALL most of what had happened.
Even though Lisa was reclining on the cloth covered couch and urinated
herself while on the CLOTH couch, the he said the urine on the floor
EXTENDED AT LEAST two feet – A LOT OF URINE!

Re: May 4, 2013 – decease of Lisa Goltman
Claimed NOTHING was out of ORDINARY that evening.
Did not recall if she was on one of the couches.
Asked if Lisa had been incontinent – he said he did not know.
He could not recall who was there –
Said Lisa NOT A THREAT TO PATIENTS,.

Lisa was assisted to stand up, WALKED PHYSICALLY from there.
Took MINIMAL EFFORT.
Any other assistance, COULD NOT SAY FOR SURE.
LISA – VERY EASILY LAID DOWN – FACE DOWN.
LEFT SIDE OF FACE TO MATTRESS.
JOSH Failed to mention TWO security guards were allegedly HOLDING LEGS
ALL THE WAY DOWN THE HALL TO THE SECURE ROOM, FOLLOWED BY AMENDRA
PRASAD, PEACE OFFCER, TEAM LEADER.
LISA RESISTANT? NOT THAT I REMEMBER.
WHAT DID OTHER PEOPLE DO? I DON’T RECALL.
WHEN ON MATTRESS? I EXITED THE ROOM – AFTER A FEW SECONDS.

DID LISA MAKE NOISES? MUMBLING.
HOW LONG? I DON’T KNOW.
DO ANYTHING ELSE? I DON’T REMEMBER.

STRONG SHEET USED TO COVER LISA.
WHAT COVERED?
TORSO.
UP TO HER NECK

“WRAPPED”. NO IT WAS NOT.

MEETING With Detective NIEHAUS – DID NOT RECALL MEETING
POLICE – DID NOT RECALL IF FEMALE OR MALE POLICE.
NIEHAUS NOTES STATEMENT OF JOSH MIDDLETON –
“SHE HAD BEEN AGGRESSIVE AND WRAPPED UP IN STRONG SHEETS”.
JOSH EXPLAINED IT WAS A “POOR CHOICE OF WORDS, SHE WAS COVERED NOT WRAPPED”.

DR. ARNOLD VOTH- Edmonton, AB – Internist / Geriatrician
Dr. Voth told the Inquiry that Propranolol was given to Lisa as Beta Blocker,
also for tachycardia, thyrotoxicosis
There was a question whether LISA was suffering from hyperthyroidism.
It was said Lithium could cause hyperthyroidism.
Linkage between hyperthyroidism and tachycardia.
March 3, 2013, DR. LEICHT prescribed Lithium bid.
This was increased on April 8, 2013 and again IN May.
May 4, 2013 she deceased.

The Inquiry will continue until April 16, 2016.
